The undersigned hereby agrees to release and indemnify the MISSISSAUGA BALLET ASSOCIATION (MBA), its employees and volunteers from any claims or lawsuits arising from injuries or accidents incurred by the applicant while participating in the dance class(es) at the Living Arts Centre, 4141 Living Arts Drive, Mississauga, Ontario.

The undersigned acknowledges that the information collected on these registration forms may be used for the following purposes: to prepare classes or group lists, collecting / processing payments, to contact parents and guardians in case of emergency or for matters related to MBA, mailing any / all information to participants and parents, for the creation and distribution of observation schedules, and for the co-ordination of volunteers for MBA-related activities. The undersigned consents to the collection and use of this information by the MISSISSAUGA BALLET ASSOCIATION. This information will NOT be released to any other organization without the written permission of the undersigned.

The undersigned hereby agrees that the MISSISSAUGA BALLET ASSOCIATION cannot be held responsible for any items that a student brings to the facility.  It is strongly recommended that students bring no valuables to classes.

WITHDRAWAL/REFUNDPOLICY 2022-23:

The MBA requests a written letter indicating the reason for withdrawal from the school. 50% of each scheduled payment (see fee chart) is non-refundable. The other 50% of the paid amount will be refunded according to the following schedule:

• Payment one:  50% is refundable until November 1, 2022.  No refunds will be given for the first installment after November 1.
• Payment two:  50% is refundable until February 15, 2023.  No refunds will be given for the second installment after February 15.
• Full payment:  If fees are paid in full at the beginning of the season, the equivalent of 50% of the first installment,  plus the second installment will be refunded if the withdrawal is on or before November 1, 2022.  The equivalent of 50% of the second installment will be refunded if the withdrawal is after November 1, but before February 15, 2023.
