Your agreement to have your child/ward participate in an online class with the Mississauga Ballet Association means that you fully agree to the following:

I understand that I have enrolled my child/ward in a program of potentially strenuous physical activity.

I hereby absolve the staff and board of directors of the Mississauga Ballet Association from any responsibility for injuries my child/ward might sustain while practicing these movements or any actions learned with or by the staff of the Mississauga Ballet Association.

I hereby release for myself, my heirs and assigns, the Mississauga Ballet Association, from any claims, demands and causes of actions arising from my child’s/ward’s participation in this program.

Agreement to this Liability Waiver will act as my agreement to all ensuing sessions, workshops and/or seminars via Zoom or any other video conferencing tool.
